OUR SERVICES


EARLY INTERVENTION

The Holmes County Board of Developmental Disabilities provides services to infants and toddlers, birth to 2 years, who have a developmental delay. The Board provides Evidence Based Early Intervention Services through Developmental Specialists, an Occupational Therapist, a Physical Therapist and a Speech Therapist. Our aim is to support families in achieving the outcomes they have identified for their child. Our staff member acts as a coach to parents in the child’s natural environment as they go through typical family routines.

FAMILY SUPPORT SERVICES

The Family Support Services program assists families caring for a loved one with a disability in their home. The purpose of the program is to provide funding for disability related goods and services which enable the individual with a disability to be an active member of the family and to remain in the family home.

FAMILY SUPPORT SERVICES PROGRAM APPLICATION

SCHOOL AGE (AGES 5-22)

The Board operates a school under the authority of the Ohio Department of Education for students in Kindergarten – Grade 12. Students who attend our school require individualized special education intervention and intensive therapies which include: Music therapy, Occupational therapy, Physical Therapy, and Speech Therapy. Our school has a full-time, on-site RN who provides medication administration, seizure monitoring, and tube feeding. The Board provides transportation to students enrolled in the school.


TRANSPORTATION

The Board provides door-to-door transportation in wheelchair accessible busses and vans to students enrolled in our School.


SERVICE & SUPPORT ADMINISTRATION

Anyone three years of age or over may request a Service and Support Administrator, SSA. An SSA assists the individual and their family, friends and care providers to develop an Individual Service Plan (ISP). The ISP is the driving force behind all of the services an individual receives. What is IMPORTANT TO and IMPORTANT FOR the individual is identified along with the individual's desired outcomes. SSAs and teams work with individuals to design and create a life that is meaningful to them.


PROVIDER SERVICES

Holmes County Board of Developmental Disabilities works with a network of providers certified by the Ohio Department of Developmental Disabilities to provide a wide variety of services to youth and adults. 


Services are funded through a combination of personal resources, Holmes County Board of Developmental Disabilities levy tax revenues, and Medicaid waivers.


When needs arise, the SSA assists the individual and their family to select a provider to meet their needs. Many services are available.


The most commonly used services are described below.

✔ Adult Service providers work with adults outside of their home. They help adults accomplish their goals, learn new skills and participate in activities of their interest. Adult service providers also can help participants learn valuable work skills, explore community employment opportunities, and find and keep a job.

✔ Transportation providers can drive individuals to appointments, shopping, recreation activities, adult service centers, jobs, etc.

✔ Residential providers are known as “homemaker/personal care” providers. They work with individuals in their home or in the community to accomplish tasks, such as cooking, cleaning, shopping, bathing, visiting friends, attending medical appointments, and much more.


WAIVER SERVICES

When an eligible individual contacts Holmes DD and requests services, Holmes DD will assess their needs. If the assessment indicates that the individual’s needs are immediate or current, Holmes DD will assist them by connecting them to resources that will meet their needs. Depending on financial resources of Holmes DD, an individual may be enrolled on a Medicaid waiver. Medicaid waivers (Individual Options, Level 1, and SELF) provide a wide variety of services including homemaker/personal care, respite, adapted equipment, environmental modifications, adult day services, and transportation to name a few. With the support of their SSA, waiver recipients choose an Ohio Department of Developmental Disabilities certified provider for each service that is needed to meet their desired outcomes. HEALTH AND WELFARE

The Holmes County Board of Developmental Disabilities is responsible to ensure the health and welfare of Holmes County residents with developmental disabilities. If you are concerned that a child or adult is being abused or neglected, or you are a provider required to report a major unusual incident (MUI) please call (330) 674-8045. ADVOCACY

The Holmes County Board of Developmental Disabilities works with teens and adults with developmental disabilities to become as independent as possible. They have an advocacy group that meets monthly to offer support to each other as they to learn how to advocate for themselves.
