Eligibility

ELIGIBILITY

The first step to determine if a person is eligible to receive Holmes County Board of Developmental Disabilities services is to call the SSA Supervisor at 330-674-8045 Ext. 224.


Who can receive services from Holmes County Board of Developmental Disabilities?

Eligible individuals must meet the following six criteria:

1.

Reside in Holmes County

2.

Have a developmental disability

3.

Disability affected them before age 22

4.

Disability is expected to be lifelong

5.

Disability results in age-specific functional limitations

6.

Person needs a combination and sequence of care/treatment/services for an extended period of time that is individually planned

BIRTH THROUGH AGE 2

Eligible individual must have at least one developmental delay or a diagnosed physical or mental condition that has a high probability of resulting in a developmental delay.

AGES 3-5

Eligible individual must have at least two developmental delays.

AGES 6 or Older

Eligible individual must have a substantial functional limitation in at least three of the following areas of major life activity: self-care, receptive and expressive language, learning, mobility, self-direction, capacity for independent living, and if the person is age 16 or older, capacity for economic self-sufficiency.

What Happens Next?

If you are interested in starting services for yourself or a family member:

  • For children birth through age 2, please call 1-800-755-4769 or make an online referral at https://ohioearlyintervention.org/referral.
  • For people ages 3 years to adult, please call 330-674-8045 and ask for SSA Supervisor. 
  • Those wishing to know if they are eligible will need to complete the Application for Eligibility Determination form and provide required medical information. Once this information is received, Holmes County Board of Developmental Disabilities will complete the assessment for limitations to determine if you are eligible for our services.
